STEVEN RIVERS
M.D.
Dr. Rivers moved to The Villages in July 2004. He was born and raised in West Chester, Pennsylvania. He completed both his undergraduate degree and medical degree at the University of Iowa located in Iowa City, IA.
He completed his internship, and residency at the Florida Hospital Family Medicine Residency located in Orlando, FL. He is board certified in Family Medicine.
Dr. Rivers is devoted to providing quality, compassionate, medical care with a focus on preventive medicine. He is aggressive in educating the patient, and involving them in the decision making process. Services include, but are not limited to: comprehensive medical care, health maintenance, minor surgical procedures, skin care, and preventive services.

M.D.
Julio Ugarte M.D. grew up in Miami, FL. He completed his undergraduate degree at the University of North Carolina at Chapel Hill. He then completed medical school at The University of South Florida in Tampa, FL.
He also completed both his internship and residency at the Florida Hospital Family Medicare Residency in Orlando, FL and is board certified in Family Medicine.
He relocated to The Villages area in 1993. He thoroughly enjoys partnering with his patients to strive for optimal health and disease prevention. He uses both conventional and functional medicine in his integrated practice.
Julio Ugarte M.D. grew up in Miami, FL. He completed his undergraduate degree at the University of North Carolina at Chapel Hill. He then completed medical school at The University of South Florida in Tampa, FL.

Dr. Dariano has been with Rivers Family Medicine for 2 years. Prior she had been practicing Family Medicine in Lima Ohio at Lima Memorial Hospital for 7 years. Dr Dariano is a board certified D.O, Doctor of Osteopathic Medicine. She completed her undergraduate degree at Bowling Green State University, and her medical degree at Ohio University College of Osteopathic Medicine.
Dr. Dariano has been with Rivers Family Medicine for 2 years. Prior she had been practicing Family Medicine in Lima Ohio at Lima Memorial Hospital for 7 years. ANTHONY DESMARAIS

Dr Desmarais completed his final year of residency at Lower Bucks Hospital in Bristol, Pennsylvania. He completed his Medical Degree at Philadelphia College of Osteopathic Medicine’s home campus in Philadelphia. Prior to this, Dr. Desmarais served in the United States Army for 8 years. He went to the highly prestigious West Point Military Academy in New York right after high school, earning his BA in Information Systems Engineering as well as his commission to the ranks of a military officer where he continued his military service, including a year-long deployment to Iraq in 2009.
Dr. Desmarais and his wife Kelly have 2 children. Dr Desmarais is committed to providing thorough, compassionate, mindful care for his patients.

We are proud to inform you that our practice offers the opportunity to use the power of the web to track the most important aspects of your healthcare through our office. The Patient Portal enables our patients to communicate with our doctors, nurses, and staff members easily, safely, and securely via the Internet using a browser or a smart phone using an application called Healow that is a free download. If you sign up you will be given your ID and password, enabling you to access the Portal and your records, including lab and diagnostic test results, educational information, billing statements, and other health information.
By using the Patient Portal, you no longer have to call the office, leave a message, and wait for a response to get the results of your lab work. Those results will be available to you through the Portal.
The Healow app is the mobile app that allows you all the same access as the portal with your mobile device.

Please arrive 30 minutes before your arrival time. Your new patient packet must be submitted to the office no less than 2 days prior to your appointment to allow time for your information to be added to your chart. Click here to complete your new patient intake form online.
To ensure that you are seen, please be sure to bring the following to your appointment:
• Original insurance cards (Photo copies will not be accepted)
• A valid government issued photo ID (Villages residence cards do not qualify)
• A current medication list that includes both dosage and frequency
• Your completed new patient packet
• For safety purposes all minor children under 18 years of age, must be accompanied by a parent or legal guardian.
